1/3/2005 Long Term Monitoring Established Reviewed Well Installations, Abandonment and Sampling Report dated 29 December 04. Slightly elevated concentrations of benzene are still present in UMW-2, located in the original source area. The concentration is considerably lower than previous sampling events and shows a continued reduction iof petroleum contamination at the source area. All other monitor wells were below ADEC cleanup standards. Donald Seagren

MW15, a down-gradient MW has only been sampled 2 times. Will require all MW's to be sampled in 2006. Benzene was detected above the groundwater cleanup levels in MW-2, MW-3, and MW-7, however all the constituents remained below the alternate cleanup levels approved for this site on February 4, 2003. 1/28/2009 Update or Other Action Reviewed the Second Semiannual 2008 Groundwater Monitoring Report, dated December 15, 2008 from Conestoga-Rovers and Associates for Paul Horwath and provided him a summary for his review and oversight. Site conditions remain stable with an overall decreasing trend in hydrocarbon concentrations. Monitoring Well UMW-15, which is located by the property boundary, has not been located during the past two monitoring events due to construction. A replacement monitoring well may need to be installed along the same property boundary if the monitoring well has been damaged or destroyed. Alyce Hughey
8/28/2009 Document, Report, or Work plan Review - other Reviewed and approved the Monitoring Well Decommissioning and Installation Workplan, dated August 7, 2009, from Conestoga-Rovers and Associates. Monitoring wells UMW-6 and UMW-5 will be decommissioned in accordance with ADEC's Monitoring Well Guidance, dated February 2009, and one additional monitoring well will be installed in the former location of monitoring well UMW-15 (UMW-15 was destroyed during 2008 road improvements along Lower Mill Bay Road. The following condition was applied to the approval to the workplan. During the decommissioning of the groundwater monitoring wells, the bottom end caps on the monitoring wells shall be knocked free of the well screens prior to the placement of grout via tremie tube. 11/30/2009 Offsite Soil or Groundwater Disposal Approved Approved the transport of one 55 gallon drum of petroleum impacted soil to Alaska Soil Recycling in Anchorage for treatment and disposal under their ADEC approved Opertion Plan. Following treatment, ASR's post-treatment sampling and analysis of this treated soil should include analytical testing for GRO, DRO, and BTEX. Paul Horwath

Monitoring well UMW-15R was installed on September 1, 2009 to replace UMW-15 at a depth of 15 feet below ground surface, and static water level was encountered at approximately 8.5 feet below ground surface within the monitoring well. No soil contamination was found above the soil cleanup levels during the development of the monitoring well. Monitoring wells UMW-5 and UMW-6 were decommissioned on September 1, 2009 due to the low hydrocarbon groundwater concentrations within the wells. Alyce Hughey

During the June 2010 sampling event five of the six monitoring wells sampled showed decreasing hydrocarbon concentrations. Monitoring well UMW-4 had an increase in Benzene concentrations to above the alternate cleanup levels. This increase in concentration is consistent to the previous spring of 2009 sampling event. On November 3, 2010 monitoring wells UMW-1, UMW-3, UMW-7 and UMW-15R were decommissioned according to ADEC February 2009 Monitoring Well Guidance. Two groundwater monitoring wells were sampled with benzene being detected above the groundwater cleanup levels in one well sampled. Seven soil borings were advanced in areas of known historical contamination with results detecting benzene above the record of decision levels with concentrations ranging from 0.4 to 8.7 mg/kg at depths of 5.5 to 8.5 feet below ground surface (ft. bgs). Residual soil contamination exceeding the migration to groundwater cleanup levels of DRO at 1,600 mg/kg at 5.5 ft. bgs, benzene at 0.2 mg/kg at 3.5 ft. bgs, and GRO at 700 mg/kg at 8.5 ft. bgs were detected in the subsurface on November 1, 2011. 10/31/2011 Update or Other Action Conditional approval was issued to discharge the purged groundwater that was collected from the 2011 groundwater sampling event to the ground surface. This request is approved, provided that there is soil surface present at this site which will accept these waters, without any discharged wastewaters running overland and leaving the site. This approval is specifically conditioned upon the requirement that all discharged wastewaters remaine onsite, and infiltrate to the surface soils onsite. No discharged wastewater shall run off the property as surface water runoff. Paul Horwath
12/12/2011 Document, Report, or Work plan Review - other Reviewed the First Semi-Annual 2011 Groundwater Monitoring and Sampling Report, dated October 20, 2011, and prepared by Conestoga-Rovers and Associates. During the April 21, 2011 sampling event the two remaining monitoring wells were sampled UMW-2 and UMW-4. UMW-2 has shown a consistent declining trend, where UMW-4 had an increase in Benzene concentrations to above the alternate cleanup levels.
