Problems/Comments

Soil and groundwater contamination resulting from leaking fuel which carried fuel from the heating oil tank to the residence. The onsite drinking water well is contaminated, diesel range organics were present below ADEC cleanup levels, benzene concentrations exceeded cleanup levels at 0.0315 mg/L. Residential drinking water wells at nearby properties were sampled and found to be free of petroleum products. The Site Assessment contained analytical results for VOC's for the onsite well, monitoring well and adjacent drinking water wells. Low levels of an unknown hydrocarbon which did not present a diesel pattern were detected in the monitoring well. PAH analysis on one soil sample did not contain any analytes above cleanup levels. Benzene and DRO are the only groundwater contaminants of concern, and those analytes were only detected above cleanup levels in samples from the onsite well. A copy of the onsite well log was provided with the file, well depth is 135 feet. The well was disconnected, and a water tank was installed at the residence prior to site transfer from PERP. Cc Claim # 02G023418, Ronnie Lee, State Farm Insurance P.O. Box 437 Dupont, WA 98327 # on all letters. A phone call from the property owner revealed that as of December 2005, the onsite well was reconnected to the residence. A water treatment system had been installed but samples to test the effectiveness of this system had not been submitted.

Action Information

Action Date Action Description DEC Staff
9/21/2005 Site Added to Database Site transferred from PERP. Fuel lines leaked causing soil and groundwater contamination. The onsite drinking water well is contaminated, adjacent properties were tested and found to be free on petroleum products. Tank and contaminated soil were removed with PERP oversight. Site assessment is complete, report is forthcoming. Kim DeRuyter
9/22/2005 GIS Position Updated GIS data collected from FNSB tax parcel maps and TopoZone Pro. High degree of confidence in the data. Kim DeRuyter
11/4/2005 Site Ranked Using the AHRM Site ranking would be much higher, driven by benzene contamination at the tap, however alternate water source was installed prior to site transfer to CS. Kim DeRuyter
11/8/2005 Update or Other Action Received call from Insurance Agent asking to be Cc'd on correspondence. Kim DeRuyter
11/30/2005 Update or Other Action Report Review - Site Assessment records soil contamination above cleanup levels remains along southwest corner and possibly beneath the garage. Soil contamination is believed to be the result of leaking couplings in the fuel lines associated with the HOT. 190 cubic yards of contaminated soil was removed and transported to OIT for thermal remediation. Two exploratory trences were dug to delineate the vertical extent of the contamination. PID readings in the trenches were below 1 ppm. Results of PAH analysis of the soil were all well below cleanup or non-detect. Groundwater results from the on-site well contain DRO contamination at or slightly below the cleanup level. Benzene results from the onsite well are above cleanup levels at 35 ug/L. A monitoring well was installed down gradient of the heating oil tank. This MW did not contain DRO or BETX analytes above cleanup levels. An alternate water source was established for the residence. Three nearby residents allowed sampling of their drinking water wells for VOCs and BETX analytes all three wells were non-detect for the listed analytes. Kim DeRuyter
11/30/2005 Long Term Monitoring Established Letter sent requesting plans for this summers groundwater monitoring. Kim DeRuyter
12/12/2005 Update or Other Action Received call from property owner. The onsite well has been reconnected to the residence. A water treatment system was installed for removal of benzene. The effectiveness of the system had not been established. I explained that the water needed to be tested by a qualified laboratory prior to being used as a drinking water source. The owner agreed to find an alternate drinking water source until the water could be tested. Kim DeRuyter
3/29/2006 Proposed Plan Received and approved the 2006 and 2007 Groundwater Sampling Plan. Kim DeRuyter
12/22/2006 Update or Other Action 2006 Groundwater Monitoring Results Recieved. BETX and DRO were not detected in the monitor well. Have not received the drinking water well results. Made a call to the property owner to discuss submitting results. Kim DeRuyter
1/5/2007 Update or Other Action Received drinking water sample results, the well was tested pre and post treatment. DRO was detected below the cleanup level in the pre-treatment sample at 1.17 mg/L. BETX analytes were also detected in this sample; benzene at 13 ug/L exceeded the clean up level of 5 ug/L. DRO and benzene were non-detect in the post treatment sample, xylenes were detected below the cleanup level. Results indicate that the treatment system is working, though continued monitoring and maintenance will be necessary to ensure that this water is safe to drink. Completed the lab quality assurance checklist - data quality is acceptable. Kim DeRuyter
4/9/2008 Exposure Tracking Model Ranking Initial ranking with ETM completed. Neal Everson
6/4/2008 Conditional Closure Approved The ground water shall continue to be treated to ensure the water meets the established regulatory levels. The treatment system shall be operated and maintained until the ground water (prior to treatment) achieves the established cleanup levels. The water (before treatment) shall be sampled for petroleum hydrocarbon contamination on an annual basis and analyzed using EPA Method 524.2. The results shall be submitted to ADEC for review in order to determine when cleanup levels are consistently achieved and treatment will no longer be required. Neal Everson
6/4/2008 Institutional Control Record Established Neal Everson
3/16/2009 Update or Other Action Sent letter requesting ICs be followed (Drinking water well needs sampled). Also, this site may need a deed notice. Neal Everson
7/24/2009 Site Visit Collected drinking water samples from 814 Gold Mine Trail and from surrounding properties to determine the current status of the on-site well and to confirm that nearby drinking water wells are not impacted. Following removal of the downgradient monitoring well, a neighbor expresed concern about the groundwater quality in the area. The property was in foreclosure until recently. The current owner is aware of the problems with the drinking water well and plans to reinstall the treatment system which froze and broke open. Janice Wiegers
8/12/2009 Update or Other Action Received drinking water results. Contamination remains in the on-site drinking water well, but no contaminants were identified in the neighboring drinking water wells. Spoke to the landowner about installing a treatment system before using the on-site drinking water well. He plans to haul in water until the treatment system is fixed. Janice Wiegers
8/31/2009 Update or Other Action Received letter from State Farm Insurance indicating that the monitoring well was decommissioned by Nortech and the insurance claim will be closed. Janice Wiegers
3/31/2011 Institutional Control Compliance Review IC review conducted, tickler system updated, and periodic review added to the IC requirements. Evonne Reese
5/9/2011 Site Visit Received call from neighboring property regarding recent odors in their drinking water well. DEC collected a sample from the neighbors drinking water well on May 3. No petroleum contaminants were detected in the well water. Janice Wiegers
8/10/2011 Update or Other Action Spoke to the current landowner regarding their drinking water well and he confirmed that they are not drinking the groundwater. The water treatment system that was used by the former owner has not been repaired. The current landowner has a water tank inside the house and it is filled using water from an offsite source and he considers this a permanent fixture in his home. He does not want his family drinking the groundwater even when filtered first. Consider removing the groundwater monitoring requirement. Evonne Reese
10/7/2011 Institutional Control Compliance Review Removal of the groundwater monitoring requirement since the landowner has a permanent water tank that is filled with water from an off site source. The groundwater is not being used as a home water source on this property, nor does the property owner plan on using it in the future. The IC Unit will check with property owner every three years to make sure site conditions have not changed. Evonne Reese
10/10/2012 Institutional Control Update The DEC project manager name has been changed from IC Unit to Janice Wiegers. Evonne Reese
12/19/2012 Institutional Control Update Drinking water sampling was performed by DEC staff at two neighboring properties. This was requested by the homeowners due to the proximity of their property to the source property. Evonne Reese
1/1/2013 Document, Report, or Work plan Review - other Received drinking water laboratory sampling results from the two neighboring properties and DRO or BTEX were not detected in the drinking water. Evonne Reese
6/6/2013 Long Term Monitoring Complete Administrative action added during a quality control check. Kristin Thompson
11/12/2014 Institutional Control Update An IC reminder letter was issued to the responsible party on this date. Kristin Thompson
5/8/2020 Institutional Control Compliance Review IC letter mailed to the landowner on this date. Site is under three year compliance review to verify they are using an offsite drinking water source. Next IC reminder will be issued in 2022 (last compliance review was due 2019). Cascade Galasso-Irish
