Statement of Contractual Terms
This document is required when contracting with primary response action contractors (PRACs). A Plan enlisting the help of a PRAC will not be approved without this contract. -
Spot Charter Amendment to an Existing Plan
A Spot Charter Amendment requires the Plan application listed above and must include all the information listed in the Spot Charter Checklist. For more information on spot charter amendments refer to the FAQ page. -

Nontank Vessels Forms

Plan Application -
Suspension of an Existing Plan
This document is required whenever a nontank plan holder intends to discontinue response contracts with its Incident Management Team and/or clean-up contractors. -
Reinstatement of an Existing Plan
This document is required whenever a nontank plan holder intends to reinstate an existing Plan. This requires the plan holder to affirm that the Plan specifics have not changed since the suspension of the Plan or that the Department has approved an update to the Plan; the Incident Management Team and clean-up contractor contracts have been activated for the Plan; and, no modifications other than reinstatement of approval are requested by the application. -
Termination of an Existing Plan
This document is required whenever a nontank plan holder intends to terminate an existing Plan. - Refer to 18 AAC 75.426 for nontank vessel streamlined plan applicability and contents.
