Nontank Vessels Forms

Nontank Vessel Streamlined Plan (used by the majority of plan holders):
  • In order to apply for a new Plan, renew an existing plan or apply for a Plan amendment, use the following form:

  • Suspension of an Existing Plan
    This document is required whenever a nontank plan holder intends to discontinue response contracts with its Incident Management Team and/or clean-up contractors.
  • Reinstatement of an Existing Plan
    This document is required whenever a nontank plan holder intends to reinstate an existing Plan. This requires the plan holder to affirm that the Plan specifics have not changed since the suspension of the Plan or that the Department has approved an update to the Plan; the Incident Management Team and clean-up contractor contracts have been activated for the Plan; and, no modifications other than reinstatement of approval are requested by the application.
  • Termination of an Existing Plan
    This document is required whenever a nontank plan holder intends to terminate an existing Plan.
