Reporting

The Compliance Program has provided the following methods for your use in meeting permit non-compliance reporting requirements, or reporting other permit issues associated with your facility. The two dedicated phone numbers are to be used for verbal notification within 24 hours of an incident of non-compliance. The required written follow-up notification can be provided via fax, email, or U.S. Postal Service. We have also provided Noncompliance and Spill Notification forms electronically below in both Word and PDF.

Discharge Monitoring Reports (DMRs) and Other Required Permit Report Submittals

Electronic reporting is highly encouraged when available. Only State Permit or Mixing Zone DMR's can be submitted by fax or email. Permittees can fax eligible DMRs to the fax number above. Permittees can fax DMRs to the fax number above or submit DMRs by using the email address above in accordance with the following guidance. Permittees must first certify the DMR in accordance with the Signatory Requirements of the permit. Certified monthly DMRs can then be scanned (in PDF format) and emailed no later than the 15th day of the following month. DEC will accept late electronic submittals. The subject line on the email should include the following: DMR for (insert month) for Facility Permit Number (insert permit number). Certified DMRs can be faxed no later than the 15th day of the following month to the fax number provided above. Late certified DMRs will be accepted. Unsigned reports transmitted via fax or email or mailed to the address above will not be accepted. The permittee must also mail the original certified DMR to the mailing address listed above.

Permittees can submit other required reports using the email address above in accordance with the following guidance. Permittees must first certify the report in accordance with the Signatory Requirements of the permit. Certified reports can then be scanned (in PDF format) and emailed no later than the specified submittal date in the permit. If the permit does not require that certified written reports also be mailed to the mailing address above, the permittee must retain the legible originals of these documents and make them available to the Department upon request. Unsigned reports will not be accepted. The subject line on the email should include the following: Report Type (insert annual for example) for Facility Permit Number (insert permit number). The permittee may also be required to submit certified copies by mail to the mailing address specified in the facility permit to fully comply with permit reporting requirements.

Permittees can now submit their AKR06000 MSGP Annual Report Form using the "MSGP Report Submittal" online application in the eNOI System. The MSGP Report Submittal application allows permittees to submit a MSGP Annual Report Form as an attachment to an online application. Online Reporting

Permittees can now submit many of their permit-required reporting online using the EDMS Water Online Application System. Please note that in order to use the Water Online Application System, you must first have or create a myAlaska account. More information is provided on the EDMS homepage.
