Problems/Comments

Diesel range organics remains in soil due to releases from a former UST. Groundwater has been sampled several times, DRO was detected slightly exceeding the cleanup level in 2002 at 1.8 mg/L in MW-5 located about 35' downgradient of the former tank. Subsequent samples detected DRO, GRO, and BTEX below cleanup levels. The building was demolished in 2009.

2/2/2010 Update or Other Action A Hazardous Materials survey was conducted prior to demolition of several old buildings on the IHS campus. PCBs were detected to 3.4 mg/kg on the floor of the generator building (Bldg 303). The building was demolished during the summer of 2009. ADEC has requested that IHS submit a sampling and analysis plan to determine the extent of the PCB contamination on the slab and surrounding soil. Kim DeRuyter
5/25/2010 Site Visit DEC made a visit to Bldg. 303 to discuss the next steps for the remediation process and to see progress made thus far. DEC suggested different sampling options with a tribe representative and a local contractor for the concrete slab. DEC recommended soil samples should be taken along the back wall of the former building and out the area where the front door was based on visual soil observations and common practice at the time the building was in use. It was also recommended by DEC that becuase of existence of PCBs the area be covered by a tarp and roped off to discourage people from entering the area. 6/22/2010 Meeting or Teleconference Held DEC(Dooley, Howard, Crapps) held a teleconference with Kathleen Peters-Zuray (TTC) and Kathryn Foster (Ridolfi) to discuss the sampling plan for the concrete slab. 7/8/2011 Update or Other Action DEC received the shipping manifest for the PCB contaminated concrete building slab from the former building 303. The slab was removed and received by Waste Management, Inc in Oregon in October 2010. The manifest notes that the excavation has been covered by a poly liner to mitigate exposure to PCB contamination in the soil below and adjacent to the former building footprint. Meghan Dooley
8/26/2011 Site Characterization Workplan Approved DEC received the draft Soil Sampling and Analysis Plan on August 17, 2011. Comments were sent on August 19, 2011. The final Soil Sampling and Analysis Plan was approved on August 26, 2011. The plan calls for using a grid sampling approach to characterize the Building 303 area for any potential PCB contamination in the soil. Additionally, the plan calls for completing soil borings at 4 different locations throughout the rest of the former hospital site to characterize petroleum contamination in the subsurface. 2/25/2015 Site Characterization Report Approved The Alaska Department of Environmental Conservation (ADEC) approved a report titled Removal Action Report for the 2012 Field Season – Zone 4, Former AANHS Hospital Site – Native Village of Tanana. The report addresses construction of a remediation treatment area (RTA), excavation of petroleum contaminated soil from Zone 4 of the AANHS Hospital project site, transport of soil to the RTA, and soil sampling as laid out in the Soil Remediation Plan. Soil sample results from the former generator building (Building 303) were below cleanup levels for PCBs, but DRO above ACLs was discovered. This soil will be targeted for removal at a later date. 6/9/2016 Site Visit DEC performed a site visit at all IHS Tanana sites and the Remediation Treatment Area. The site status for each area was discussed with IHS and Tanana representatives. Monte Garroutte
3/1/2019 Update or Other Action Note this LUST site is being managed contemporaneously with the CS site "IHS Tanana Health Center" as one large site with multiple source areas as explained in a memo from former PM Colin Craven to former Program Manager Kim DeRuyter, dated March 21, 2008. On March 1, 2019 I reviewed a letter and Interim Report from Ahtna with ISM sample results form the RTA. Results indicate that the top 10-inches of soil (a tilled lift) in the northern half of the landfarm (Decision Unit [DU]-1) meets the SRTLs and cumulative risk does not exceed permissible levels. To allow for unrestricted use, SRTLs are the most stringent 18 AAC 75.341 Method Two, Tables B1 and B2 Soil Cleanup Levels (SCLs). On August 20, 2108, ADEC approved the transport of approximately 1,200 cubic yards (CY) of soil from the landfarm to the borrow pit staging area for reuse by the City of Tanana. The southern half of the landfarm (DU-2) did not meet the SRTLs and will need additional tilling and fertilizer application. On March first I contacted the RP (Tanana Tribal Council; Kathleen Peters-Zuray) introduced myself as the current PM and requested the Operations, Maintenance and Monitoring (OM&M)Report for 2017-2018 and updates on Plans for 2019 work. This Covenant subjects the Property to certain activity and use limitations including restrictions on groundwater use and excavations, and notification requirements for construction activities. Description Details
Groundwater Use Restrictions No groundwater wells shall be installed on the contaminated area of the Property without prior DEC approval.
Groundwater Monitoring Contaminated groundwater underlying the Property may not be pumped, drained, or dewatered; or used for irrigation, dust control, or any other purpose without prior DEC approval. If that use is approved under this Covenant, that use is still subject to all applicable treatment, monitoring, disposal, and permitting requirements.
New Construction Restrictions Any construction, modification, or use of buildings on the contaminated area of the Property is prohibited without prior review and approval by DEC.
New Construction Restrictions If the use of a building on the Site changes, or if buildings are constructed within 30 feet of the Site, DEC must be notified and may require that Grantor complete an evaluation of vapor intrusion and mitigation of any vapor intrusion risks.
Excavation / Soil Movement Restrictions Excavation, drilling, and other intrusive activities below a depth of 5 feet are prohibited within the Site, without prior review and approval from DEC.
When Contaminated Soil is Accessible, Remediation Should Occur In the event that contaminated soil on the Property becomes accessible in the future, the Grantor shall notify DEC, characterize the contamination, and, if determined necessary by DEC, cleanup the soil pursuant to DEC's Site Cleanup Rules.
Subdivision Restriction DEC must be notified in advance of any subdivision or replat of the Property. This Covenant must be included as part of future Property transactions and attached to subsequent associated parcels, as determined applicable by DEC.
Advance approval required to transport soil or groundwater off-site. DEC approval is required prior to moving soil or groundwater where contamination remains above applicable default cleanup levels (Table C or Method Two under current regulations). If DEC approval for movement is granted, any moved soil or groundwater must still be characterized and managed following regulations applicable at that time.
Movement or use of contaminated material (including on site) in a manner that results in a violation of the water quality standards is prohibited (18 AAC 70)
Groundwater Use Restrictions Groundwater throughout Alaska is protected for use as a water supply for drinking, culinary and food processing, agriculture including irrigation and stock watering, aquaculture, and industrial use. Contaminated site cleanup complete determinations are based on groundwater being considered a potential drinking water source. In the event that groundwater from this Site is to be used for other purposes in the future, such as aquaculture, additional characterization and treatment may be required to ensure the water is suitable for its intended use.
