Incident Description

Location Prudhoe Bay, Kuparuk Oil Field, Drill Site (DS) 1J, Well house 137.
Product/Quantity CPAI estimated a spill volume of 710 gallons of produced water, released from a line supplying artificial lift fluid to a production well. CPAI reported that the crude oil concentration of the produced water is estimated to be 1,000 ppm, equaling 0.71 gallons of crude oil spilled.
Cause A flow meter failed due to erosion as produced water injection came back on-line following pigging operations.
Time/Date of Incident Approximately 1:00 p.m. on April 21, 2012. ConocoPhillips Alaska, Inc. (CPAI) reported the spill to ADEC at 1:33 p.m.
