1. Review Guidelines for Administering Provisional Exams Outside the Exam Cycle.
2. The two-page Provisional Application is to be completed at the course. Course attendees are NOT to complete the application prior to the exam and submit it to staff at the Operator Training and Certification Program (OpCert). Provisional applications are to be returned with the Provisional exams at the completion of the course.
3. Exam Request Forms must be received by staff fourteen (14) days prior to the course start date. Please include a current roster of course registrants. Late requests will not be accepted. The request forms may be mailed, faxed, or emailed to OpCert staff.
4. Course Timeline:
- Minimum of 30 days prior to the course/exam date, the course must be listed on the Alaska Training Coalition Calendar and be approved by OpCert staff.
- Fourteen (14) days prior to the course start date submit the Exam Request Form to OpCert staff.
- OpCert staff will arrange for a proctor for the exam.
- Immediately following the completion of the exam, the proctor will mail back all exam materials and Provisional applications to OpCert staff.
- Upon receipt of the exam materials, OpCert staff will invoice the course provider for application and exam fees.
- Upon receipt of fees, exams will be processed and results mailed out.
