How to Obtain a Replacement Certificate

Order an Official Replacement Certificate

By Mail

  • Complete the Request for Certificate Replacement form.
  • Mail the form to the Operator Training and Certification Program with the $50 per certificate replacement fee. Checks should be made payable to the "State of Alaska."
  • Please allow 1 - 3 weeks to process your request.

Online

  • With a myAlaska account, you can pay the $50 certificate replacement fee by accessing the online fee payment page.
  • Once you access the payment page:
    1. "Fee" column: Select "Replacement Certificate ($50)"
    2. "Operator Name" column: Enter full name
    3. "Operator City/Sate": Enter city and state. Please access your online operator profile and ensure that your mailing address is correct.
    4. "Notes" column: Enter the certificate type and level.
    5. Enter contact information at the bottom of the page and click on the "Continue to Payment Page" to enter your credit card information.
    6. Email your payment receipt to dec.opcert@alaska.gov.
    7. Please allow 1 - 3 weeks to process your request.

Print an "Unofficial" Copy of Your Certificate

If your myAlaska account is connected to your online operator profile, you can download an "unofficial" copy of your certificates.

  • Access your online operator profile by clicking on the "Manage Your Operator Profile" button.

  • Once you access your online operator profile by using your myAlaska credentials, click on the "Certification" tab.

  • On the Certification page, click on the certificate number you would like to print.

  • You will be able to download a low resolution "unofficial" two-page copy of your certificate as a PDF file.
