The manufacturer of the device may be found on the device (label or plaque) or in the owner’s manual.
The model of the device may be found on the device (label or plaque) or in the owner’s manual.
The Serial Number of the device may be found on the device (label or plaque) or in the owner’s manual.
The Year the device was manufactured may be found on the device label. If unknown, choose unknown. An unknown year of manufacture will default the device as EPA uncertified.
If the emission rating is specified, the emission rate unit must be selected. The emission rating of the device may be found on the device label. For wood stoves the emission rating is usually in grams per hour or g/hr. For hydronic heaters, the emission rating is usually in pounds per million BTUs or lbs/mmBTU. After recording the number, please indicate the appropriate unit. If unknown, the default will be a value for an uncertified device which is greater than 7.5 g/hr or greater than .32 lbs/mmBTU. This is a free form decimal number field, enter "." for a decimal number. Leave it blank if unknown.
Please put in the year that the device was installed at its current physical location.
