2/2/2005 Institutional Control Record Established ADEC Decision The information presented to date indicates that a major source of petroleum contaminated soil has been excavated and transported off site for treatment. There was a small amount (2 cubic yards) of impacted soil remaining on site. ADEC has determined that no further remedial action will be required at this site subject to the following conditions: 1. Any proposal to transport soil and/or groundwater off site requires ADEC approval in accordance with 18 AAC 75.325(i); 2. An institutional control (ADEC database entry) will identify the nature and extent of the contamination remaining on the site. It will also notify future owners/operators of the property of the environmental status of the site and any conditions that apply to future management of the contamination; and 3. This determination is based on current information. Additional investigation and/or cleanup action may be necessary in the future if new information indicates there is hazardous substance contamination at this site that may pose a risk to human health and the environment. 4. Final site closure will be considered following confirmation that all soil above established cleanup levels has been removed and properly treated. Sarah Cunningham
2/2/2005 Conditional Closure Approved 2 cubic yards of impacted soils remain onsite above ADEC cleanup levels. NFRAP letter sent to Ken Soundy Sarah Cunningham

5/1/2013 Institutional Control Record Removed The conditions at this site meet the 2009 closure policy, therefore ICs can be removed. The default requirement of no offsite transport of soil or groundwater will still apply. In accordance with 18 AAC 75.380(d)(2), ADEC may require additional site assessment, monitoring, remediation, and/or other necessary actions at this facility should new information become available that indicates contamination at this site may pose a threat to human health or the environment.
