Conditional Closure Approved |
Phorwath issues a December 13, 2006 conditional closure decision letter. Conditions of closure include prohibition of water well installation without prior ADEC approval, ADEC must be contacted if residual soil or groundwater contamination is exposed due to future site work and additional site assessment or cleanup may be required at that time, long-term annual groundwater monitoring from MW-8, and the decommissioning of the remaining GW monitoring wells. |

Institutional Control Record Established |
This data base entry will serve as an institutional control to document that residual soil and groundwater contamination remains at this site at concentrations exceeding the most stringent 18 AAC 75.341 soil 'Migration to Groundwater' cleanup levels and the 18 AAC 75.345(Table C) groundwater cleanup levels. |

Document, Report, or Work plan Review - other |
Phorwath reviewed MWH's Tesoro 2 Go Mart #54 October 2006 Monitoring Event Report Final, dated February, 2007. Three GW monitoring wells were sampled. Residual BTEX, GRO, and DRO was detected but no groundwater cleanup levels were exceeded in any of the three wells. Only GW MW-8 is required to be preserved for future annual long-term GW monitoring. |

Document, Report, or Work plan Review - other |
Reviewed MWH's Tesoro 2 Go Mart #54 February 26, 2008, Monitoring Wells Decommissioning. All monitoring and treatment wells were decommissioned with the exception of MW-1, which will continue to be periodically sampled in order to monitor long term groundwater quality, in accordance with the conditions of the site closure determination. |

Document, Report, or Work plan Review - other |
Reviewed the May 2007 Drinking Water Well Monitoring Report, dated June 2008, from MWH and provided Paul Horwath a summary for his review. Based on the May 2, 2007 drinking water sample, no DRO, GRO, and BTEX groundwater contamination was encountered during this monitoring event. Site conditions remain stable. |

Document, Report, or Work plan Review - other |
Reviewed the April 2008 Monitoring Event Report, dated July 2008, from MWH, for Paul Horwath and prepared a summary of the report for his review and oversite. Benzene, Ethylbenzene, DRO and GRO was detected in the groundwater sample taken, but were below the ADEC groundwater cleanup levels. |

Document, Report, or Work plan Review - other |
Reviewed the April 2009 Monitoring Event Report, dated June 2009, from MWH for Paul Horwath and prepared a summary for his review and oversight. Site conditions are stable with only GRO and DRO being detected below the groundwater cleanup levels in the one well tested SVE-1/MW. The on-site drinking water source continues to test non-detect for all analytes tested for. Continued monitoring of the one remaining well, SVE-1/MW and the on-site drinking water source is needed in order to confirm the absence of contaminants in the groundwater and in the stations drinking water source. |

Long Term Monitoring Complete |
Issued letter for ADEC Removal of Long-Term Groundwater Monitoring Requirement. Upon review of the groundwater quality data collected at this site since issuance of our December 13, 2006 ADEC Record of Decision, we have determined that long-term groundwater monitoring is no longer necessary. Condition number 3 in ADEC's December 13, 2006 ADEC Record of Decision - Conditional Closure Determination is hereby removed, meaning that further groundwater monitoring and sampling is no longer required at this site. The status of this site will remain "Cleanup Complete with Institutional Controls" on the ADEC database. An institutional control will remain on the ADEC database to document residual contamination potentially remaining at the site. A site closure without conditions could be considered when soil and groundwater samples confirm that all soil and groundwater meet the established ADEC soil and groundwater cleanup levels for this site. |

Document, Report, or Work plan Review - other |
Reviewed the Release Investigation Letter Report, dated August 30, 2010, fromm MWH. On July 17, 2010 approximately 15 gallons of gasoline fuel was released from a boat being towed by a customer’s truck onto the pavement that migrated downhill and into soil near the western entrance to the site. Approximately 50 gallons of contaminated soil was excavated in an area of 4.5 by 1.8 by 1.8 feet and transported to Alaska Soil Recyclers for thermal remediation. Two soil samples were taken at 1.7 feet below ground surface, and soil samples did not detect GRO or BTEX above the soil cleanup levels. |
