Sample from east side, north wall of excavation on 7/7/88, had 921 ppm TPH. Seven soil samples taken from the excavation had less than 20 ppm TPH. |

No free product observed in the eight monitoring wells at the site. Groundwater is about 13 feet below the surface. Quarterly monitoring recommended by Hart Crowser. |

Holiday has renumbered their stores as: 5001=601; 5002=602; 5005=605; 5005=605; 5006=606; 5007=607; 5008=608; 5010=610; 5014=614; 5016=616; 5018=618; 5024=624; 5025=625; 5030=630; 5031=631; 5050=650.
While Holiday will oversee the work at the former Williams’ stations, these sites remain the responsibility of Williams. The numbers of these sites remain the same: 5003; 5004; 5009; 5012; 5015; 5017; 5021; and 5034. |

ADEC has determined that the cleanup actions employed at WES #5015 were effective in removing a majority of the contaminant source material and a conditional closure is appropriate for the site. Based on the information provided to date, it has been determined that the contaminant concentrations remaining on site do not pose an unacceptable risk to human health or the environment and no further remedial action is required.
This determination is subject to the following conditions:
1. A deed notice (Notice of Environmental Contamination) shall be recorded in the State Recorder’s Office that identifies the nature and extent of contamination at the property and any conditions that the owners and operators are subject to in accordance with this decision document. A copy of the recorded notice shall be provided to ADEC within thirty (30) days of its recordation
2. An institutional control will be recorded on the ADEC database that identifies the nature and extent of the contamination remaining on the site.
3. ADEC shall be notified prior to off site transport of soil or groundwater in accordance with 18 AAC 78.274(b).
4. Annual groundwater monitoring shall be required in accordance with an ADEC approved monitoring plan. Monitoring wells MW-3, MW-4, MW-7, and MW-9 will be sampled for benzene, toluene, ethylbenzene, and xylenes.
5. Monitoring well MW-5 and any other unnecessary groundwater monitoring wells shall be decommissioned in accordance with accepted practices.
6. Groundwater wells will not be installed on this property without prior approval from ADEC. |

Made first attempt to contact Walgreens regarding construction/development activities at this location, which was formerly a Johnson Tire Service Store (CC in 1998). The new address is 2839 Minnesota Drive and includes the former properties from the adjacent strip mall. Twenty supersacks of material were observed at this location and CSP had not been contacted about plans to move soil. The excavated material was later determined to be associated with the former Johnson Tire Service facility and approval was obtained from Bill O’Connell to transport and dispose. |
