Action Information
Action Date | Action | Description | DEC Staff |
---|---|---|---|
3/11/1993 | Update or Other Action | UPD; Site assessment 9-11-93; site assessment report 11-12-93; report submitted 3-8-93. Six months from discovery to notification. | Former Staff |
3/11/1993 | Underground Storage Tank Site Characterization or Assessment | SA1R; Gibler: stockpiling plan not approved. Excavation was lined with 20 mil liner and dirt was placed in hole, covered with liner, then more dirt. Bottom of hole is in zone of seasonal water table fluctuation. GRPH = 2190 bottom of fill end of hole. BTEX = 1458 ppm total. Full extent of contamination unkown. Water not sampled. | Former Staff |
3/11/1993 | Leaking Underground Storage Tank Release Confirmed - Petroleum | LUST Site created in CSP for source area ID 76955 ADD; | Former Staff |
3/11/1993 | Site Added to Database | Former Staff | |
4/13/1993 | Leaking Underground Storage Tank Cleanup Initiated - Petroleum | LCAU; Removal of a 1000-gallon diesel and a 1000-gallon unleaded gasoline UST with the dispenser island and piping. About 45 cubic yards of gasoline contaminated soil was removed. The excavation was lined with a 20 mil liner and the contaminated soil replaced into the excavation for passive remediation. : LCAU date changed DB conversion | Former Staff |
4/19/1993 | Update or Other Action | NOR; NORL sent to Gen. Cox at request of Capt. Beck. Apparently this will facilitate the funding process for remediation. | Former Staff |
9/11/1993 | Leaking Underground Storage Tank Emergency Response | IA; Gibler: 45 cu. yds. of contaminated soil stockpiled on site, but not in an ADEC approved plan. Tank, which may have been leaking, is removed. | Former Staff |
2/14/1994 | Update or Other Action | RPL2; Peterson sent PRP-CS Database Notification Letter to RP requesting update and more environmental information concerning the contaminated site. RP rec'd letter, but no response from RP as of this "complete date". | Former Staff |
2/15/1995 | Release Investigation | RP conducted a Release Investigation. Six soil borings were drilled to determine the vertical and horizontial extent of the contamination found during to the UST closures. Borings were converted to temporary piezometers and ground water samples were collected. All soil samples were ND for BTEX and GRO. Water samples indicated low levels of BTEX in all samples, with P-5 & P-6 exceeding MCL's for Benzene (13 & 7.7 ppb) and P-2, P-4, P-5, & P-6 having exceedances for TPH. Groundwater was encountered at approximately 11 feet bgs | Former Staff |
8/30/1995 | Update or Other Action | Corrective Action. RP hired contractor to remove and bag the approximately 45 cy of contaminated soil found during the UST closure. During the removal process additonal contamination was found below the liner in the are a of the diesel UST. RP requested the contractor to remove all contamination greater than category "C" cleanup levels ( matrix score sheet). Contractor removed approximately 270 cy of petroleum contaminated soils and stored it in 277 one-ton super sacks for remediation at a later date. | Former Staff |
9/10/1998 | Update or Other Action | ADEC PM issue closure on the soil contamination associated with the gasoline and diesel releases from the two USTs. Soil was treated to category "A" cleanup levels using an Enhanced Thermal Conduction and Vapor Extraction System. System also treated non-UST contaminated soils from the AK ANG AAOF facility. | Former Staff |
8/22/2000 | Site Closure Approved | ADEC PM issued a NFA determination letter after reviewing two annual groundwater sampling events indicating the groundwater had remediated below MCLs (ND) for constituents of concern. | Former Staff |
12/5/2002 | Update or Other Action | ADEC PM changed from Tim Stevens to Egbejimba Beatrice. | Beatrice Egbejimba |
12/5/2002 | Update or Other Action | ADEC PM received e-mail with information regarding well decommissioning activities from the AKARNG PM. PM reports that ICRC is under contract to the Alaska Army National Guard to decommission four monitoring wells at the former Kotzebue OMS UST site. This activity is scheduled for the week of 8-14 December 2002. This is the last activity related to the site closure. | Beatrice Egbejimba |
2/4/2004 | Update or Other Action | ADEC received a letter report dated October 2003 from Norm Straub with information regarding the decommission of four monitoring wells at the Kotzebue OMS. PVC well casing was removed, the borehole was backfilled with hydrated Pure Gold bentonite chips to with in approximately two feet . The remaining two feet was backfilled with native soil. | Beatrice Egbejimba |
5/10/2004 | Update or Other Action | File number changed from L71.07 to 410.26.010 | Sarah Cunningham |
