Problems/Comments

Ordnance or ordnance-related problems. This AOC is located adjacent to and northeast of ALDA-01 and covers 9.5 acres. The site was used for ordnance disposal at the end of WWII. A relatively straight line of deep craters marches through the middle of the site, the craters are the remnants of cratering charges that were set along the former roadway to prohibit access to the disposal areas deeper into ALDA-02. EOD performed removal in area in mid 60's and recovered numerous MEC. Steep slopes, tall grass, thick at most locations, impedes access and hides holes and hummocks in the uneven terrain. Clearance of ALDA-01 may extend past the boundary into parts of ALDA-02. Munitions Debris (MD) was observed during site walk in 2012 though no MEC has been observed along this portion of the seawall. The site was determined to be NOFA in the RI/FS due to inaccessibility and lack of recent MEC finds.

While the GIS mapped a large portion of this site as inaccessible a narrow strip of land along the shoreline is indeed accessible. The Navy extended a transect through this area and representative areas were excavated to determin the nature of debris/munitions in this area. 3/7/2012	CERCLA FS	Submitted approval letter to the Navy for the Final OUB-2 Feasibility Study. ALDA-02 is not currently included in the Remedial Action Area identified in the document. ALDA-02 had be preliminarily assigned NOFA status based on limited site information. The Navy recently completed a Historical Photographic Analysis for OUB-2 and this effort identified additional information that indicates ALDA-02 was used for terrestrial ordnance disposal. The FS identifies this site as requiring further evaluation prior to determining whether remedial actions are required at this site. 11/7/2014	Update or Other Action	Site walk in 2012 did confirm that the western extent of ALDA-02 was used for ordnance disposal. Documentation from 1960's indicate this area was cleared of MEC by Navy EOD. Munitions Debris was identified in this area. However the location meets several criteria for inaccessibility (slope, cobbles, and craters). MEC was not observed and MEC routinely identified along the seawall does not appear to originate from this location. Clearance of ALDA-01 scheduled for 2016 will extend into ALDA-02 as required. 3/29/2019	CERCLA Remedial Action Completion Report	This report documents APTIM’s results for the 2018 season. The scope of this work was to remove MEC and/or material potentially presenting an explosive hazard (MPPEH) from areas within the RAA-01 (open burn/open detonation) and RAA-03W (remaining grids adjacent to RAA-01) and the RAA-05 Andrew Lake Disposal Area (ALDA). MEC/MPPEH were removed by conducting excavations of metal saturated areas, removal of discrete digital geophysical mapped (DGM) anomalies via hand digging assisted by mini excavators, and mag and dig in a small number of areas outside the DGM survey area (step out areas). For metallic saturated areas, APTIM used armored, long-reach excavators to remove material and spread it into lifts, which were subsequently inspected for MEC/MPPEH by teams of unexploded ordnance (UXO) technicians using handheld electromagnetic metal detectors. For discrete DGM anomalies, APTIM reacquired the anomalies with survey equipment and used handheld metal detectors and removed the anomalies via mechanically-assisted hand digging. With the assistance of handheld metal detectors and mini-excavators, the step-out areas were cleared via hand digging. APTIM also performed monthly seawall sweeps for MEC/MPPEH along the Andrew Lake Seawall during the months that the APTIM crews were working on Adak.	Darren Mulkey
3/29/2019	CERCLA Removal Action Report	Received the Final Completion Report 2018 Field Season Operable Unit B-2, Non-Time Critical Removal Action, Remedial Action Areas -01, -03W, and -05, Former Naval Air Facility Adak, Alaska date March 2019. The Navy began conducting NTCRA activities in 2013. In years 2013 and 2014, the Navy completed, with regulatory approval, the removal actions in RAA-02, RAA-03 (East), and RAA-04. The three remaining in-process OU B-2 sites were RAA-01, RAA-03 (West), and RAA-05. The Navy continued NTCRA activities in the final three RAAs during the 2015 through 2018 field seasons. 3/25/2020	CERCLA Remedial Action Completion Report	Under the Comprehensive Environmental Response, Compensation, and Liability Act and the Federal Facilities Agreement, the U.S. Department of the Navy (Navy) is required to complete necessary munitions and explosives of concern (MEC) removal actions for site areas within Operable Unit (OU) B-2 on Parcel 4 of the former Adak Naval Complex, located at Adak Island, Alaska. See Figure ES-1 for an overall site layout and completion status. OU B-2 contains five remedial action areas (RAA). The Navy is performing the required removal activities under a non-time critical removal action (NTCRA) to satisfy the remedial action objectives identified as the preferred remedy from the Proposed Plan for Operable Unit B-2 Cleanup of Munitions and Explosives of Concern, Former Naval Air Facility Adak, Adak Island, Alaska (Navy, 2012). Upon completion, it is the Navy’s intention to return the property to the U.S. Department of the Interior. Currently, OU B-2 is in the removal action investigation phase of the Comprehensive Environmental Response, Compensation, and Liability Act process. The Navy began conducting NTCRA activities in 2013. In years 2013 and 2014, the Navy completed, with regulatory approval, the removal actions in RAA-02, RAA-03 (East), and RAA-04. The three remaining in-process OU B-2 sites at that time were RAA-01, RAA-03 (West), and RAA-05. The Navy continued NTCRA activities in the final three RAAs during the 2015 through 2018 field seasons. By the completion of the 2018 season, removal actions at RAA-01 and the remaining RAA-03 (West) grids were completed, leaving RAA-05 as the only incomplete site. This report documents Aptim Federal Services, LLC (APTIM)’s results for the 2019 season at RAA-05 Andrew Lake Disposal Area -02 (ALDA-02) and Andrew Lake Seawall (ALSW). The scope of this work was to remove MEC and/or material potentially presenting an explosive hazard (MPPEH) from areas within RAA-05 ALDA-02 and ALSW. MEC/MPPEH were removed by conducting excavations of disposal areas, and surface clearance in the areas adjacent to the disposal areas. For disposal areas, APTIM used armored, long-reach excavators to remove material and spread it into lifts, which were subsequently inspected for MEC/MPPEH by teams of unexploded ordnance technicians visually using screening/sorting tables or with handheld electromagnetic metal detectors on wooden search pads. For surface clearance areas, APTIM cleared these areas via hand digging of metallic items at the surface. During the months that the crews were working on Adak, APTIM also performed monthly seawall sweeps for MEC/MPPEH on the cobble beaches on ALSW and adjacent to ALDA-02.	Darren Mulkey
3/26/2020	Document, Report, or Work plan Review - other	Quality Assurance Surveillance Report. The QA field effort for the 2019 field season involved three QA personnel on island. A total of 4,956 man hours were expended during the field season. QA personnel conducted 135 field surveillances and 1,632 trench/lift inspections. Thirty-eight QA BSIs were emplaced in the trenching operations and 38 QA BSIs were emplaced in the surface clearance areas. All QA BSIs were successfully recovered by the production contractor. The QA team verified 154 surface clearance grids completed by the production contractor as complete. One NCR was issued by QA during the 2019 field season; the corrective action was successfully completed by the production contractor to remedy the issue. No major safety incidents occurred during the field season. Overall, the production contractor successfully implemented the processes delineated in the QAPP and other pertinent project documents. For metallic saturated areas, APTIM used armored, long-reach excavators to remove material and spread it into lifts, which were subsequently inspected for MEC/MPPEH by teams of unexploded ordnance (UXO) technicians using handheld electromagnetic metal detectors. For discrete DGM anomalies, APTIM reacquired the anomalies with survey equipment and used handheld metal detectors and removed the anomalies via mechanically-assisted hand digging. With the assistance of handheld metal detectors and mini-excavators, the step-out areas were cleared via hand digging. APTIM also performed monthly seawall sweeps for MEC/MPPEH along the Andrew Lake Seawall during the months that the APTIM crews were working on Adak.

CERCLA Removal Action Report

Received the Final Completion Report 2018 Field Season Operable Unit B-2, Non-Time Critical Removal Action, Remedial Action Areas -01, -03W, and -05, Former Naval Air Facility Adak, Alaska date March 2019. The Navy began conducting NTCRA activities in 2013. In years 2013 and 2014, the Navy completed, with regulatory approval, the removal actions in RAA-02, RAA-03 (East), and RAA-04. The three remaining in-process OU B-2 sites were RAA-01, RAA-03 (West), and RAA-05. The Navy continued NTCRA activities in the final three RAAs during the 2015 through 2018 field seasons.

CERCLA Remedial Action Completion Report

Under the Comprehensive Environmental Response, Compensation, and Liability Act and the Federal Facilities Agreement, the U.S. Department of the Navy (Navy) is required to complete necessary munitions and explosives of concern (MEC) removal actions for site areas within Operable Unit (OU) B-2 on Parcel 4 of the former Adak Naval Complex, located at Adak Island, Alaska. See Figure ES-1 for an overall site layout and completion status. OU B-2 contains five remedial action areas (RAA). The Navy is performing the required removal activities under a non-time critical removal action (NTCRA) to satisfy the remedial action objectives identified as the preferred remedy from the Proposed Plan for Operable Unit B-2 Cleanup of Munitions and Explosives of Concern, Former Naval Air Facility Adak, Adak Island, Alaska (Navy, 2012). Upon completion, it is the Navy’s intention to return the property to the U.S. Department of the Interior. Currently, OU B-2 is in the removal action investigation phase of the Comprehensive Environmental Response, Compensation, and Liability Act process. The Navy began conducting NTCRA activities in 2013. In years 2013 and 2014, the Navy completed, with regulatory approval, the removal actions in RAA-02, RAA-03 (East), and RAA-04. The three remaining in-process OU B-2 sites at that time were RAA-01, RAA-03 (West), and RAA-05. The Navy continued NTCRA activities in the final three RAAs during the 2015 through 2018 field seasons. By the completion of the 2018 season, removal actions at RAA-01 and the remaining RAA-03 (West) grids were completed, leaving RAA-05 as the only incomplete site. This report documents Aptim Federal Services, LLC (APTIM)’s results for the 2019 season at RAA-05 Andrew Lake Disposal Area -02 (ALDA-02) and Andrew Lake Seawall (ALSW). The scope of this work was to remove MEC and/or material potentially presenting an explosive hazard (MPPEH) from areas within RAA-05 ALDA-02 and ALSW. MEC/MPPEH were removed by conducting excavations of disposal areas, and surface clearance in the areas adjacent to the disposal areas. For disposal areas, APTIM used armored, long-reach excavators to remove material and spread it into lifts, which were subsequently inspected for MEC/MPPEH by teams of unexploded ordnance technicians visually using screening/sorting tables or with handheld electromagnetic metal detectors on wooden search pads. For surface clearance areas, APTIM cleared these areas via hand digging of metallic items at the surface. During the months that the crews were working on Adak, APTIM also performed monthly seawall sweeps for MEC/MPPEH on the cobble beaches on ALSW and adjacent to ALDA-02.

Quality Assurance Surveillance Report. The QA field effort for the 2019 field season involved three QA personnel on island. A total of 4,956 man hours were expended during the field season. QA personnel conducted 135 field surveillances and 1,632 trench/lift inspections. Thirty-eight QA BSIs were emplaced in the trenching operations and 38 QA BSIs were emplaced in the surface clearance areas. All QA BSIs were successfully recovered by the production contractor. The QA team verified 154 surface clearance grids completed by the production contractor as complete. One NCR was issued by QA during the 2019 field season; the corrective action was successfully completed by the production contractor to remedy the issue. No major safety incidents occurred during the field season. Overall, the production contractor successfully implemented the processes delineated in the QAPP and other pertinent project documents. The work completed by the production contractor met all of the criteria for acceptance during the final QA checks.
