Problems/Comments

The site MM-10F was created in September 2004 to bound the primary impact area within MM-10E. Portions of MM-10A and MM-10B were also included in this new area. The site is contaminated with unexploded ordnance and associated munitions debris. Investigation of this site occurred in 1999, 2000, 2002, and 2004. Remediaiton is scheduled to begin in 2008 and be complete by the end of 2009.

6/3/2008 CERCLA Remedial Design/Remedial Action Plan Approved Submit work plan approval letter to Navy for OUB1 Project Plans. Plans identify Remedial Action to be implemented at the remaining OUB-1 Sites (MM-10F, MM-10G, and MM-10H). Work is anticipated to take 2-3 field seasons. Guy Warren
9/25/2009 Meeting or Teleconference Held Meeting with Navy and EPA to discuss numerous instances of Non-conformance with work plan requirements. Since the beginning of the 2009 field season the Navy has restricted communication with agency staff (ADEC and EPA) limiting participation in weekly meetings and not providing QC documentation in a timely manner. Twenty-seven (27) Non-conformance Reports (27) have been issued by the QA contractor identifying deficiencies in field work. Many of these NCR’s remain unaddressed several months after issue and the documentation was not provided to agencies for review until one week prior to the meeting. The agencies discussed the NCR’s their implications on the field effort and possible re-work that could address the issue. The team identified additional data collection that needed to be accomplished to verify the adequacy of the removal conducted to date. Guy Warren
12/14/2009 Meeting or Teleconference Held Meeting with Navy and EPA to discuss 2009 field season and resolve Non-Conformance Report (NCR) issue. The contractor presented the results of the additional data collection effort the occurred at the end of the field season. Munitions Debris was identified in a small percentage of the completed excavations that were evaluated. Contractor QC and Government QA not in agreement on the facts surrounding the NCR’s and the MD remaining in some of the excavations. However, no MEC was identified in any completed excavation even though some MD was left in place. Plans for the 2010 field season will include backfilling the excavations remaining on site and restoration of ruts and other erosions issues. Guy Warren
5/18/2011 CERCLA Remedial Action Completion Report Submit comments to Navy and their contractor on the OUB-1 AOC’s (MM-10F, MM-10G, and MM-10H) Remedial Action Project Documentation. In general the first three sections of the report are well written and present the data from the 2008, 2009, and 2010 field seasons. However the report fails to adequately address the Non-conformance Reports (NCR) issued by the Navy’s quality assurance (QA) contractor to the production contractor (Section 4). The NCR resolution document continues to leave many of the NCR’s unresolved. The lessons learned document fails to identify the true cause of many of the NCR’s and appears to be an attempt to absolve Navy Project Management of any responsibility for the failure to follow the approved work plans during the 2009 field season and the resulting uncertainties over whether the remedial action objectives have been achieved. ADEC recommended significant revisions to sections 4 and 5 of the report to address the non-conformance. Guy Warren
8/29/2012 Meeting or Teleconference Held Meeting with Navy and EPA to discuss agency comments on the OUB-1 AOC’s (MM-10F, MM-10G, and MM-10H) Remedial Action Project Documentation (RAPD) Report. This document was originally submitted for agency review in early 2011 and comments were provided to the Navy at that time. The new Navy PM has been trying to wrap things up with the 2008-2010 OUB-1 Field efforts and provided a response to ADEC’s and EPA’s previous comments. The project team agreed to remove the “lessons learned” section of the document (section 5). In addition we reviewed and revised the Navy’s responses to ADEC and EPA comments. All comments were resolved adequately and all parties agreed to finalize the four part RAPD. Guy Warren

2/22/2013 Cleanup Complete Determination Issued By law, DEC is required to recover expenses incurred during cleanup, including staff oversight time. Current and former landowners may be liable for state cleanup expenditures. Issue "Cleanup Complete with Institutional Controls" determination to the Navy for MM-10A, MM-10B, MM-10E, MM-10F, MM-10G, and MM-10H. Completion of ROD required remedial actions documented adequately in the "Remedial Action Project Documentation" report. No site specific IC's will be applied to the sites, they will fall under the Island Wide Ordnance Awareness/Education program that is currently being implemented by the Navy. Specific activities include: Provide UXO-related information to island residents, (Including: maps, UXO awareness video, coloring book with Boomer the otter, posters etc.), Coordinate with local school to ensure twice annual presentation to children, IC awareness video playing at airport during flight arrival and departure, Verification of contacts (on-island and EOD response units), Maintenance of signs and fences, and Periodic Inspections.
