Report an Illness or Issue

Contact the Food Safety and Sanitation program to report foodborne illness, waterborne illness, and food or sanitation issues at food establishments and public facilities.

For illness or issues associated with facilities in the Municipality of Anchorage (Anchorage, Eagle River, and Girdwood), please visit the Anchorage Health Department's Environmental Health Services website or call their Food Safety and Sanitation Program office at 907-343-4200.

Reporting is Helpful

Reporting issues that you experience helps the Department of Environmental Conservation protect public health. Your report can help identify foodborne and waterborne illness outbreaks in Alaska, highlight areas where operators may need resources and intervention, and prevent the spread of illness to others.

All illnesses and issues reported to the Department of Environmental Conservation are investigated.

Reporting a Foodborne or Waterborne Illness

  • Foodborne illness often occurs one to four days after eating contaminated foods
  • Waterborne illness usually occurs two to 14 days after contact with contaminated water
  • After a report is received, Department staff will follow up with a phone call to complete an illness questionnaire
    • An illness questionnaire takes about 20 to 30 minutes to complete
    • The questionnaire includes questions about symptoms, foods that were recently eaten and their source, and any recent water activities
    • All information collected about an illness is private; the Department will not release names to anyone outside the public health system without permission
  • Reports can be made in three ways:
  • Reporting lines are staffed Monday through Friday from 8 a.m. to 4 p.m. (not including holidays)
  • Messages are confidential; please include a name and daytime telephone number so the Department can follow up on the report
