AQ Compliance Forms
Listed below are the commonly used compliance forms required for reporting. Please refer to your permit for a complete list of compliance forms required and when they are due.

Methods for submission:
- Email to: DEC.AQ.Airreports@alaska.gov ; or
- Mail to: Alaska Department of Environmental Conservation, Air Compliance Program, 610 University Avenue, Fairbanks, Alaska, 99709; or
- Fax: 907-451-2187; or
- Phone notifications: 907-451-5173. Phone notifications require written follow up report; or
- Online submission can be made through the Air Online System (AOS). Report must be submitted by an authorized E-Signer and responsible official for the stationary source. Information and assistance obtaining the E-signer role and authorization may be found at AOS Help website.
Online submission will be required via this method starting September 7, 2023 in accordance with 18 AAC 50.270. (c).

If submitted online, report must be submitted by an authorized E-Signer for the stationary source. An E-signer is a responsible official who has been identified and authorized for electronic submissions. Information and assistance obtaining the E-signer role and authorization may be found at: http://dec.alaska.gov/Applications/Air/airtoolsweb/AosHelp
Note: A myAlaska account is needed to gain access, and a profile needs to be set up in the Permittee Portal. You do not have to be an Alaska resident to set up a myAlaska account or a Permittee Portal profile. Submission will be required via this method starting September 7th 2023 in accordance with 18 AAC 50.270. (c) which states the following: For records and information that must be submitted using electronic web forms that are available before September 7, 2022, unless an owner or operator is already required to submit records or information electronically, the requirement to use the forms takes effect no later than September 7, 2023
