EDMS Construction General Permit (CGP) Annual Report

Instructions for completing the APDES Construction General Permit (CGP) Annual Report form for stormwater discharges associated with construction activity under the CGP using EDMS.

Permit Information

Enter contact information for the entity that operates the project described in this application. Enter the operator's telephone number, mailing address, and e-mail address.

Report Details

Enter project name.

Enter complete street address, including city, state, zip code, and county or similar government subdivision of the project. Do NOT use a P.O. Box. If the project lacks a street address, indicate the general location of the site (e.g., Intersection of State Highways 61 and 34).

Answer Yes or No: do you have substantially identical discharge points on a linear project as described in Part 7.3.4 of the ACCP

If you do have substantially identical discharge points on a linear project, list identical discharge point names or ID numbers which are identified in your SWPPP that are not sampled but visually monitored.

Indicate if the discharge was a result of a rain event or snowmelt.

If the discharge was the result of rainfall, provide the total amount of rain for the storm event in inches.

Indicate if the rainfall measurement was taken using an onsite gauge or a National Weather Service precipitation gauge.

Enter valid date on which samples were collected.

Monitoring Results

All discharge points on your site subject to monitoring shall have two turbidity samples collected, averaged, and reported as average downstream turbidity. Compliance is determined based upon the difference between the individual upstream sample for that specific discharge point and the average downstream turbidity result.

Complete one set of tables for each storm event (rainfall or snowmelt) that resulted in a discharge from the site. At a minimum per part 7.3.2.2 of the CGP two samples per discharge point shall be collected and averaged.

Identify all receiving water bodies or wetlands to which the project's stormwater discharged.

Indicate if any of your stormwater discharges from construction activities reached a 303d listed water (i.e., impaired water body). For a listing of impaired waters and an interactive map, see https://dec.alaska.gov/water/water-quality/integrated-report .

If the stormwater discharge reached a 303d listed water, enter the pollutant causing the impairment.

Per Part 3.2, Upstream monitoring must take place at a representative location (upgradient) from the point of discharge or outside the area of influence.

  • Provide the date and time the Upstream samples were collected.
  • Enter the latitude and longitude for the Upstream sample locations>
  • Enter the measured turbidity for the Upstream samples in Nephelometric Turbidity Units (NTUs).

Per Part 3.2.1, Downstream monitoring must take place at a representative location inside the area of influence or at the point where the storm water discharge leaves the construction site.

  • Provide the date and time the Downstream samples were collected.
  • Enter the latitude and longitude for the Downstream sample locations>
  • Enter the measured turbidity for the Downstream samples in Nephelometric Turbidity Units (NTUs).

Enter the average of the two samples collected from each discharge point.

The difference between the upstream and average downstream sampling results from each discharge point will be calculated and displayed in Nephelometric Turbidity Units (NTUs).

Signing

This section allows you to either sign the form or send an email invitation to have someone else sign the form.

Check box "Me" if you are signing the form. Select the "Sign" button to continue. (You may also select "Finish Later" to continue submission to a later time.) The next page presents the Signature requirements for permit applications and reports (18 AAC 83.385). If you agree to the terms, check the box and select โ€œSignโ€. You may also select "Cancel".

Check box "Someone Else" if you will be sending an email invitation to have someone else sign the form. Enter the user's email who will sign the form, including optional instructions. Select "Send Invite". (You may also select "Finish Later" to continue submission to a later time.) The next page gives you the opportunity to "Cancel Signing" or "Rescind Invite".

At the time of submission, the form will be transmitted to DEC, and it will become part of the public record.
