EDMS STORM WATER ENGINEERING PLAN REVIEW
Instructions for completing an application for a Storm Water Engineering Plan Review – Letter of Approval for stormwater discharges associated with construction activity using EDMS.
Contact Information
Enter contact information for the Applicant (Project Owner or Project Sponsor), and Applicant (Engineer or Engineer of Record). Enter the name of the contact person and the legal name of the firm, public organization, or any other public entity that is responsible for this project. Select role(s) for each contact. Multiple roles may be selected per contact.
Project Information
Enter the estimated project start date and estimated project completion date.
Enter complete physical address, including city, state, and zip code of the project. Do NOT use a P.O. Box. If the project lacks a street address, indicate the general location of the site (e.g., Intersection of State Highways 61 and 34). Complete project location information must be provided for a Strom Water Engineering Plan Review - Letter of Approval to be granted.
Select the correct borough for the project location.
Enter the latitude and longitude of the project site in decimal degrees format with up to 6-digit accuracy. You can obtain the project's latitude and longitude through EDMS Map, Global Positioning System (GPS) receivers, internet map service, U.S. Geological Survey (USGS) quadrangle or topographic maps, or EPA's web-based siting-tools, among other methods. For consistency, DEC requests that measurements be taken from the approximate center of the project site.
Select the method used to determine geographic coordinates.
Enter the estimated total project area (acres). Indicate to the nearest tenth of an acre. Note: 1 acre = 43,560 sq. ft.
Enter the estimated area (acres) to be disturbed including but not limited to grubbing, excavation, grading, and utilities and infrastructure installation. Indicate to the nearest tenth of an acre. Note: 1 acre = 43,560 sq. ft.
Indicate whether the project has been previously covered by an EPA or APDES DEC permit. If “Yes”, enter the permit authorization number that the project was covered under.
Discharge Information
Identify all receiving water bodies or wetlands to which the project's stormwater will discharge. These should be the first bodies of water that the discharge will reach. For example, if the discharge leaves your site and travels through a roadside swale or a storm sewer and then enters a stream that flows to a river, the stream would be the receiving water body. Waters of the U.S. include lakes, streams, creeks, rivers, wetlands, impoundments, estuaries, bays, oceans, and other surface bodies of water within the confines of the U.S. and U.S. coastal waters. (Waters of the U.S. do not include man-made structures created solely for the purpose of wastewater treatment.) U.S.G.S. topographical maps may be used to make this determination. If the map does not provide a name, use a format such as “unnamed tributary to Cross Creek”.
Indicate if any of your stormwater discharges from construction activities will reach a 303d listed water (i.e., impaired water body)? For a listing of impaired waters and an interactive map, see https://dec.alaska.gov/water/water-quality/integrated-report/.
Enter the estimated distance from above waterbody to the project site (feet).
Plan Review Checklist
This section provides a checklist for required information to be granted a Strom Water Engineering Plan Review - Letter of Approval. Please answer “Yes” or “No”. (Note: all fields are required.)
Use the ‘Comment’ field for each item to give brief details supporting the “Yes” or “No” answer.
Required Documents
This section will allow you to electronically submit required supporting documents:
- a cover letter/report describing the project and providing narrative responses to each of the checklist items
- signed and sealed engineering plans for review
- signed and sealed stormwater calculations for review

This section will allow you to electronically submit any supporting documents.
Review
This section allows you to review the form to confirm the form is populated completely and accurately, prior to signing and submission.
Signing
This section allows you to either sign the form or send an email invitation to have someone else sign the form.
Check box "Me" if you are signing the form. Select the "Sign" button to continue. (You may also select "Finish Later" to continue submission to a later time.) The next page presents the Signature requirements for permit applications and reports (18 AAC 83.385). If you agree to the terms, check the box and select “Sign”. You may also select "Cancel".
Check box "Someone Else" if you will be sending an email invitation to have someone else sign the form. Enter the user's email who will sign the form, including optional instructions. Select "Send Invite". (You may also select "Finish Later" to continue submission to a later time.) The next page gives you the opportunity to "Cancel Signing" or "Rescind Invite".
At the time of submission, the form will be transmitted to DEC, and it will become part of the public record.
