- NSHC employees discovered the spill outside the hospital at 4:11 a.m. on June 20, 2014. A local resident who detected a fuel odor from the hospital reported the spill to ADEC at 5:10 a.m. on the same day.

- NSHC estimates that approximately 2,528 gallons of home heating oil (#1 diesel) were spilled. NSHC reported that the fuel leaked from above ground storage tank #2, which has no lined secondary containment.
- Cause
- A valve actuator on tank #2 failed while the tank was being filled, causing it to overflow.
