In order to apply for initial CS-LAP approval, download and complete the Part 1 PDF form and Part 2-New Lab spreadsheet. A renewing laboratory must submit the Part 1 PDF form, and may choose to use the Part 2-Renewal spreadsheet to request automatic renewal of the lab's currently approved scope and any requested updates (addition or removal of analytes), or may instead complete a Part 2 - New Lab spreadsheet with all desired analytes.

Note for mobile labs: Mobile labs, also known as field labs, are sometimes used at remote sites to avoid the cost and logistics of shipping numerous samples to a fixed (permanent) lab. If the mobile lab is analyzing samples for compliance with state regulations, the mobile lab must be approved by the CS-LAP. However, if the mobile lab is only generating screening data to help guide field work it may only require approval in the site workplan. These decisions are made on a site-specific basis and should be discussed with the DEC contaminated sites project manager.
