The following information must be submitted to DEC as an amendment package at least five working days before the vessel enters Alaska state waters. Application package must identify specific checklist items. 18 AAC 75.415(c).

A cover letter signed by an authorized person that includes information in 1a-1f.

Written assurance that all vessel specific items in Plan Holder’s Prevention Plan have been addressed. 18 AAC 75.450

Amendment Application [18 AAC 75.408(a)(1)] Under Facility Type, select Tank Vessel or Tank Barge and select Spot Charter Vessel. The applicant is the Person with Operational Control. 18 AAC 75.400(a)(2)
